Hawaii Pacific Health Nurse Practitioner, Diabetic Care - Community Education in Honolulu, Hawaii

Job Description

Founded in 1921, Straub Medical Center is a fully integrated nonprofit health care system with a 159-bed tertiary care hospital in Honolulu, a network of neighborhood clinics and a visiting specialist program that reaches throughout the state of Hawai‘i. With more than 300 employed or contracted physicians who are leaders in their fields, Straub provides its patients with expert diagnoses and treatments for more than 30 different medical specialties, including bone and joint, heart, cancer, endocrinology/diabetes, family medicine, gastroenterology, geriatric medicine, internal medicine, women’s health, vascular and urology. Straub is home to the Pacific Region’s only multi-disciplinary burn treatment center. The hospital consistently brings new technologies and innovative medical practices to Hawai‘i, such as pioneering minimally invasive cardiac surgery and total joint replacement. In 2010, the American Heart Association recognized Straub in three areas of excellence: Coronary Artery Disease (Gold Award), Heart Failure (Silver Award) and Heart Attack (Bronze Award.)

Straub's widely recognized Diabetes team includes a group of trained endocrinologists who evaluate, diagnose and treat illnesses resulting from abnormalities of hormone-producing glands, such as the pituitary, thyroid, adrenal, parathyroid, gonads or pancreas. These health care professionals also treat patients suffering from problems related to osteoporosis, cholesterol problems and diabetes. In partnership with our primary care physicians, the team is focused on providing the best in care for both our pre-diabetic and diabetic patient populations.

If you enjoy working with people and believe in warm, compassionate health care, this is an excellent job for you! As an expert in diabetes care, your role as a Nurse Practitioner will care for our diabetes patients by assessing patient needs, supporting efforts around diabetes self-management education, and monitoring and addressing best health practices to optimize patient outcomes and quality. As a member of the Internal Medicine Department, you will work collaboratively with our clinic team and physicians to establish priorities and assess solutions for patients to ensure they receive the best care possible. We are looking for someone dedicated and experienced, with excellent communication skills and a commitment to creating a healthier Hawaii.
